Pathways Care and Support Services LLC operates fully licensed group care homes for individuals with developmental and intellectual disabilities. Specifically, the Company offers residential care services, including room and board, in addition to separate, in-home care. We are a health and social care provider with over six years experience in supporting adults with learning disabilities, mental health diagnoses, and autism spectrum disorder in supported living accommodations and group homes.

Independence

We will ensure that every person supported achieves a level of independence commensurate with their ability. We will provide tailored and person-centered support that promotes independence without compromising safety considerations.

Choice

We will actively seek to give our clients (and their families) a voice to determine how they live their lives. This includes a choice of their preferred support workers where possible, what they eat, how they dress and present themselves, their entertainment, their learning and personal development.

Empowerment

We will provide regular opportunities for clients to input into the running of their services. We will ensure that feedback loops are in place that demonstrate how client input is leading to changes in our services. We will support clients in presenting their own needs, wants and preferences to various stakeholders, including GPs, Commissioning Managers, social workers and others.

Integrity

We will engender and nurture a values-based culture that is open, honest, trustworthy, relational and personable. We will do this, in part, by adopting values-based recruitment and comprehensive staff induction to ensure that we live out our values and attributes across our entire business. This includes in our client-facing work, internally as staff and externally in our interactions with all stakeholders.

Our Clients and Licensing

Pathways Care and Support Services LLC's target client base comprises predominantly Medicaid-funded individuals between the ages of 18 and 64 and some privately funded clients. Pathways Care and Support Services LLC is registered with and licensed by the Agency for People with Disabilities (APD). We are also members of the ARC of Florida.

Our Facilities

The Company’s facilities will be set up as comfortable and spacious bungalows that can accommodate up to six residents. Via these facilities, Pathways Care and Support Services LLC offers round-the-clock care, including providing room and board, assisting with daily activities such as taking medication, bathing, dressing, eating, and facilitating transportation to medical appointments.
